A worker is fatally injured in an explosion at a chemical plant.  There is no obvious explanation but key documentation is missing.  Asked to investigate, the chemist Dr Nessa Mullane discovers metal fragments close to the blast site that contradict the emerging consensus that the accident was caused by operator error.  Certain that a mistake is being made she takes a closer look at the circumstances of the blast - a course of action that brings her into bitter conflict with plant management.
